Training records on <br /> former employees shall be kept for at least three years from the date the employee last worked at the <br /> facility. Immediately provide employees with initial or refresher hazardous waste training_ Within 30 <br /> days (June 19, 2010) provide this office with a copy of the roster and the syllabus which meet the <br /> requirements of 66265.16 CCR (California Code of Regulations). <br /> #60 Failed to keep signed copy for 3 years. <br /> #61 Failed to determine the status of hazardous waste when manifest copy not received. <br /> #62 Failed to file an exception report. <br /> At time of inspection generator copy of manifest# 000585316SKS, 1/25/08 was seen without the TSDF {{ <br /> copy_ Hazardous waste generators must maintain copies of TSDF-signed manifests for 3 years and i <br /> have them readily available for review_ If the generator did not receive a copy of the manifest with the <br /> handwritten signature of the owner or operator of the facility to which the generator's waste was <br /> submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br /> shall contact the transporter and/or the owner or operator of the designated facility to determine the <br /> status of the waste_ If within 45 days, from the date that the hazardous waste was accepted by the <br /> initial transporter,the generator did not receive a copy of the manifest with the handwritten signature of <br /> the owner/operator of the designated facility, the generator shall submit to DTSC a legible copy of the <br /> manifest, with some indication that the generator has not received confirmation of delivery. Within 30 <br /> days (June 19, 2010) provide the office with a copy of the TSDF copy or the exception report submitted <br /> to DTSC. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($115). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE.
