REHS <br /> Linda REHS <br /> M 0, 2011 Phone: (209) 468-3420 <br /> Fax: (209) 464-0138 <br /> Jasbir Chadda nnpl <br /> 1100 Main Street <br /> Manteca, CA 95337 <br /> Re: Main Street ARCO AMPM SR0062239 <br /> 1100 Main Street, Manteca <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit for a 10 feet piece of electrical wiring replacement on the 91 tank <br /> level monitor(TLM) without a permit, and also perform a cold start after a software upgrade on <br /> the underground storage tank (UST) system at the above noted address. The permit has been <br /> approved with the following conditions: <br /> Note that the TLM was tested during the facility's annual monitoring certification <br /> performed on 3/31/11. <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Muniappa Naidu at (209) 468-3439 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). Please be <br /> aware that inspections for emergency work must be scheduled within five days of permit <br /> approval. <br /> a. Sensor functionality: Functionality of the monitoring system must be verified to <br /> ensure that monitoring has been restored to meet the requirements of Title 23 <br /> California Code of Regulations and the Health & Safety Code. A copy of the alarm <br /> history shall be printed prior to performing the "cold start" of the Veeder Root system <br /> and made available at the time of EHD inspection. <br /> Please be aware that once the cold start is performed the facility cannot operate <br /> the UST system prior to the EHD inspection and approval. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br />
