According to the maintenance records on site, the 89 relay was replaced on December 1, 2012 and <br /> the 87 probe wiring was replaced on April 21, 2013; permits were not obtained from the EHD for this <br /> work. Emergency repairs relating to UST system monitoring and release detection may be performed by <br /> a properly licensed, trained, and certified contractor pursuant to the EHD's policy and a permit obtained <br /> within one business day from the EHD. All other repairs must have a permit submitted to and approved <br /> by the EHD prior to beginning work. Ensure that necessary permits are obtained for all future repairs. <br /> The positive shutdown and probe functionality were verified at the time of inspection. <br /> REPEAT VIOLATION <br /> 314. The external enunciator used as part of the overfill prevention system is not currently functional for <br /> the ATGs. Immediately discontinue deposition of petroleum into all tanks until the component is repaired <br /> or replaced by a properly licensed, trained, and certified contractor under permit and inspection of the <br /> EHD.
