Alarm log for alarm on December 28, 201 t (L 11, Disp 2-5) were not found on site. These records <br /> shall be kept on site for at least 3 years. Immediately locate and maintain the record for the alarm on <br /> 12/28/11 on site and submit copies to the EHD within 30 days of receiving this report. <br /> 204. The designated operator failed to check and note the correct test dates for the monitoring <br /> certification and leak detector test on the 12/11, 1/12, 2/12, and 3/12 designated operator monthly <br /> inspection reports. It was 6/22/11 and should have been 3/31/11. During the monthly inspection, the <br /> designated operator shall check that all required testing and maintenance for the UST system have been <br /> completed and document the dates they were done. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. Provide <br /> proof of corrective action to this office within 30 days of receiving this report. <br /> 305. The 89 , sump sensor failed to activate an audible and visual alarm when tested to ensure <br /> continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an <br /> audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. The sensor <br /> was replaced and retested and passed during the inspection (model # 794380-323, SN# 261509). <br /> Obtain a permit from the EHD for this work within one business day. If a permit is not obtained within <br /> one business day, a "Failure to Obtain a Permit" penalty will apply. <br /> 310. Liquid was observed in the 89 fill sump. If water could enter into the secondary containment by <br /> precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, <br /> make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it <br /> accordingly. Ensure that the all the sumps is maintained free of liquid.
