Subsequent to <br /> a failed precision _est on the regular gas tank, repair work <br /> to the product and vapor return pipelines was noted during <br /> the site inspection. To date, this department has not <br /> received the precision test data sheets, nor were plans for <br /> the repair work submitted for review and/or approval. The <br /> construction company responsible for the repair should <br /> contact our office within five days of the date of this <br /> letter so that permit requirements can be satisfied. <br /> The application for monitoring well construction submitted <br /> to our department by Harding Lawson Associates does not <br /> address the unauthorized release and possible contamination <br /> associated with the regular gas tank's failed precision <br /> test. As a result, we are returning the application so that <br /> a revised and more accurate work plan may be prepared <br /> addressing the area surrounding the tank farm. <br /> In addition, the San Joaquin Local Health District fee <br /> schedule Resolution No. 85-58, a $35.00 consultation fee <br /> shall be submitted along with the completed general <br /> Environmental Health Application Form for review of the <br /> above referenced environmental assessment/remediation <br /> activity. <br /> Each additional hour over the initial one hour of time spent <br /> in review activity will be billed at the rate of $35.00 per <br /> hour. The additional amount due will be billed to the <br /> applicant identified on the general Environmental Health <br /> Application Form at the completion of requested activity, or <br /> monthly, if longer than 30 days is required for completion. <br />
