Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Jeff Carruesco, R.E.H.S. <br />San Joaquin County Environmental Health Department (EHD) performed a routine underground storage tank <br />inspection on April 10, 2006 at the above referenced site. <br />An inspection report was issued to you along with a request for pertinent information that was necessary in order to <br />bring this site into compliance. You were required to submit this information by May 10, 2006. <br />Failure to correct the violations will result in continuing/additional violations. Failure to submit the completed <br />underground storage tank information IMMEDIATELY may result in further legal action. <br />The missing information include(s); <br />❑ Current State Form <br />❑ Current State Form B <br />❑ Monitoring and Response Plan <br />❑ Annual Line Tightness Test Results <br />171 and <br />Certification of financial responsibility <br />and letter from the chief financial officer <br />❑ Submit statement of designated operator <br />171 inspections statement monthly designated operator <br />inspections <br />❑ Annual Line Leak Detector Results <br />❑ Annual Monitoring Certification <br />10 Maintenance and Monitoring Log <br />❑ Pennit submitted to repair/replace failed equipment <br />❑ Submit secondary containment test results <br />❑ Subrnit spill bucket test results <br />❑ Subrnit designated operator personnel training <br />records <br />Upon inspection, liquid was observed in the 87 fill and piping sumps. Due to the fact that the UST system located <br />at this facility was constructed post Julyl, 2003, the Health & Safety Code Section 25290.2 (c)(3) requires this <br />facility to have the secondary containment be constructed to prevent any water intrusion into the system by <br />precipitation, infiltration, or surface runoff. Submit a scope of work to make this repair prior to making the <br />proper repair to this office IMMEDIATELY.
