You were required to submit this information by May 10, 2006.
Failure to correct the violations will result in continuing/additional violations. Failure to submit the completed
underground storage tank information IMMEDIATELY may result in further legal action.
The missing information include(s);
❑ Current State Form
❑ Current State Form B
❑ Monitoring and Response Plan
❑ Annual Line Tightness Test Results
171 and
Certification of financial responsibility
and letter from the chief financial officer
❑ Submit statement of designated operator
171 inspections statement monthly designated operator
inspections
❑ Annual Line Leak Detector Results
❑ Annual Monitoring Certification
10 Maintenance and Monitoring Log
❑ Pennit submitted to repair/replace failed equipment
❑ Submit secondary containment test results
❑ Subrnit spill bucket test results
❑ Subrnit designated operator personnel training
records
Upon inspection, liquid was observed in the 87 fill and piping sumps. Due to the fact that the UST system located
at this facility was constructed post Julyl, 2003, the Health & Safety Code Section 25290.2 (c)(3) requires this
facility to have the secondary containment be constructed to prevent any water intrusion into the system by
precipitation, infiltration, or surface runoff. Submit a scope of work to make this repair prior to making the
proper repair to this office IMMEDIATELY. This ofce may require a permit for the system alteration.
If you have any questions, please call me at (209) 468-9847.
Sincerely,
Michelle Le, REHS
Senior Registered Environmental Health Specialist
