a. One of several permissible valid, current contractor's licenses issued by <br /> the Contractors State License Board (may be issued to the installation <br /> company), and <br /> b. All valid, current California UST certificates issued by the International <br /> Code Council (ICC), that are applicable to the work being performed, and <br /> c. A valid, current certificate of training from the manufacturer(s) of the <br /> component(s)being tested,repaired, or serviced. <br /> And in addition <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 5. Permit expires 180 days from date of approval. Note: If an extension to the <br /> expiration date is required, a written request must be submitted to this office prior <br /> to the expiration date. <br /> 6. All components must be certified by an independent testing organization and <br /> labeled as required by the organization. (UL,ULC,ANSI, API, ASTM, etc. See <br /> California Code of Regulations (CCR),Title 23 Chapter 16 Table B for complete <br /> list). The component must bear the appropriate mark as required by the <br /> independent testing organization. <br /> 7. Any changes in information and scope of work must be submitted to EHD for <br /> approval prior to commencing work. All new addenda supersede the previous <br /> ones. <br /> 8.
