Western Fiberglass Co - <br />Flex piping will be installed. The permit has been approved with the following conditions: <br />I. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Contact Garrett Backus to arrange the following inspections (a 48 hour notice is <br />required): <br />a. A primary line and secondary line pressure and soap test to verify the diesel <br />pipeline integrity. Isolate and pressurize the new diesel pipe following the <br />manufacturer's guidelines. <br />b. 24-hour hydrostatic test or equivalent ori the diesel turbine sump and the under <br />dispenser containment. <br />3. Qualified service technicians must possessor work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(&) being tested, repaired, or serviced, and <br />d. Each person installing Or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />4. Qualified installers must possessor work under the direct and personal supervision of an <br />individual physically present at the work site, at all times while work is being performed, <br />who possesses all of the following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), and <br />b. All valid, current California UST certificates issued by the International Code <br />Council (ICC), that are applicable to the work being performed, and <br />Page 1 of 2 <br />
