A / Santa Rosa, CA 95403 <br />Ph: (800) 688-3375 / Fx: (800) 700-1188 <br />www.westemfg.com <br />Octagon Sumps <br />CERTIFICATION REQUIREMENTS <br />A contractor must be certified by a representative of Western Fiberglass to install <br />octagon sumps. <br />To obtain certification there are three options available: <br />Option I <br />Western Fiberglass will do a full installation of all octagon sumps on-site. <br />Please call our sales departments for scheduling. <br />Option 2 <br />Western Fiberglass will install (1) octagon sump and provide on-site certification <br />of contractor (Includes: travel, labor, (1) full install of tank sump, certification of <br />contractor). The contractor is responsible for installing all remaining octagon <br />sumps per factory instructions. <br />Please call our sales departments for scheduling. <br />Option 3 <br />Western Fiberglass will train contractor at an agreed upon location off-site. <br />Please call our sales department for scheduling. <br />Page 7 of 11 <br />
