Pacific Ave., Stockton, CA 95207 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to replace penetration fittings in all STP sumps, fill sumps, and UDCs on the <br /> underground storage tank (UST) system at the above noted address. The permit has been approved <br /> with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Stacy Rivera at (209) 468-3440 to arrange the following inspection appointments (a <br /> 48-hour notice is required for inspection appointments). Please be aware that inspections for <br /> emergency work must be scheduled within five days of permit approval. <br /> a. Overfill prevention verification: Measurement of drop tubes to verify 95% tank volume shut <br /> off. <br /> b. Verification of communication of secondary piping into all UDCs and sumps, as required for <br /> detection of a leak at the earliest opportunity. <br /> c. 24-hour hydrostatic test (or equivalent) on STP sumps, fill sumps, and UDCs <br /> d. If concrete will be broken near the sumps or piping, a secondary containment test must be <br /> performed on the affected portions once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> Please be aware that the facility cannot receive fuel in the wastewater tank prior to the <br /> EHD inspection and approval. <br /> 4. Secondary containment testing for the STP sumps, fill sumps, and UDCs are required six <br /> months after installation. Provide 48 hours notification to the EHD when this test is scheduled. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be issued <br /> to the installation company), or a Tank Tester's license issued to the individual by the State <br /> Water Board, and <br /> 1868 E.
