Specific well abandonment procedures included the following: <br /> • The.locked vault box and the locked 6-inch security casing were removed. <br /> • The 2-inch diameter PVC well casing was removed and the 8-inch diameter <br /> borehole was drilled out in-order to remove all,the material present in the original <br /> borehole. <br /> • The MW-5 borehole was backfilled with neat cement grout consisting of 95% <br /> cement and 5% bentonite. <br /> • Per your request, the MW-1 through MW-4 boreholes were backfilled with neat <br /> cement grout consisting of 100% cement. <br /> A copy of the well abandonment permit is included in Attachment 2. The original well logs for- <br /> MW-1 through MW-5 are included in Attachment 3. <br /> We have received a copy of the San Joaquin County Public Health Services June 20, 1995 letter <br /> confirming completion of site investigation and remedial action for the underground storage tank <br /> at the site (see attachment 4).
