PHS/EHD has a contract with <br /> California State Water Resources Control Board to conduct an underground storage tank <br /> corrective action program. The above referenced site was placed in the PHS/EHD Local <br /> Oversight Program (LOP) in October 1990. <br /> i <br /> On October 26, 1995 you met with the PHS/EHD Technical Review Committee (TRC). At <br /> i that meeting, we reviewed and discussed your Site Closure Evaluation Report. The report <br />! submitted to the TRC had inaccuracies and chronological inconsistencies. The TRC had <br /> questions regarding site assessment work and remediation work you could not answer <br /> satisfactorily. <br /> The TRC has determined that the following items must be addressed and submitted for review <br /> before this site can be considered for closure: <br /> An accurate site map including the areas overexcavated in relationship to wells and <br /> a <br /> soil borings. <br /> i <br /> A cross section depicting residual soil contamination':with mass volume calculations. <br /> i <br /> An accurate account of the VES including calculations and/or contours of the zone of <br /> influence and operation times. <br /> Site Closure certification occurs only after reporting requirements are met and all wells are <br /> f properly destroyed under permit from PHS/EHD.
