SANJ O A Q U I N Environmental Health Department <br /> COUNTY <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> MAXIM CRANE WORKS 2373 E Mariposa Rd, Stockton May 21, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> Eight drums of waste oily solids and two drums of waste oil filters (greater thsn 500 pounds; actual weight was not <br /> available) were found on site that have not been reported. Any material that meets or exceeds the reportable <br /> quantity shall be reported in the facility's business plan. Immediately log into the California Environmental Reporting <br /> System(CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to <br /> the EHD for approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, storm and sewer drains, access and exit points(structures), emergency <br /> shutoffs, hazardous material handling and storage areas(oxygen, acetylene, hazardous waste solids), and <br /> emergency response equipment in the areas of the repair shop and outdoor oil storage (fire extinguishers and spill <br /> control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and <br /> sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling <br /> and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then <br /> list it on the map and label as"NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. The plan does not contain procedures for the immediate <br /> notification of the Local Unified Program Agency, San Joaquin County Environmental Health Department. A <br /> business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> -Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS) annually by January 15 for at least 2016, 2017, 2018, and 2019. The hazardous materials inventory <br /> shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the <br /> previous year.
