The following is an itemized list of hazardous materials violations that have not been <br /> addressed for MAXIM CRANE WORKS as of November01, 2019. <br /> Open violations from May 21, 2019 inspection <br /> Violation#5 -Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Eight drums of waste oily solids and two drums of waste oil filters (greater than 500 pounds; actual weight was not <br /> available) and a tank of hydraulic oil (greater than 55 gallons; actual volume was not available)were found on site <br /> that have not been reported. Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#6 -Failed to complete and/or submit a site map with all required content. <br /> The map does not show loading areas, storm and sewer drains, access and exit points (structures), emergency <br /> shutoffs, hazardous material handling and storage areas(oxygen, acetylene, hazardous waste solids, and the tank <br /> of hydraulic oil inside the repair shop), and emergency response equipment in the areas of the repair shop and <br /> outdoor oil storage (fire extinguishers and spill control). A site map shall contain a north orientation, loading areas, <br /> internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation <br /> staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map <br /> element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California <br /> Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, <br /> and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation #7 -Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were lacking. The plan does not contain procedures for the immediate <br /> notification of the Local Unified Program Agency, San Joaquin County Environmental Health Department A <br /> business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> - Immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> - Evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
