The facility has a balance of 1,003.40 <br /> dollars that has not been paid to SJC EHD. Fees shall be paid once a year at the time of submittal of an inventory <br /> form. Immediately pay the annual fee to the EHD Accounting Office located at 1868 E Hazelton Avenue, Stockton, <br /> CA 95205. Fee may also be paid online at www.sjcehd.com (Convenience fee may apply). <br /> This is a minor violation. <br /> 2 HSC 25505, 25508(a)(1)Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> This is a Class II violation. <br /> 3 HSC 25507 Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented. The facility has 3,000 pounds of <br /> Sulfur without a Hazardous Materials Business Plan .A Hazardous Materials Business Plan shall be established <br /> and implemented for emergency response to a release or threatened release of a hazardous material when storing <br /> hazardous materials at or above the thresholds quantities of 55 gallons for materials that are liquids, 500 pounds for <br /> solids, or 200 cubic feet for compressed gas, at standard temperature and pressure. Immediately log into the <br /> California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the required information, and <br /> submit for review by the EHD. <br /> This is a Class II violation. <br /> 4 CCR 2652(a)(1) Failed to complete and/or submit the Business Activities or Owner/Operator Identification page. <br /> The Business Owner/Operator Identification form was not submitted. The facility has no HMBP plan submitted to <br /> CERS. This information must be current and submitted as part of the business plan. Immediately log into the <br /> California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> 3,000 pounds of Sulfur were found on site that have not been reported. Any material that meets or exceeds the <br /> reportable quantity shall be reported in the facility's business plan.
