EHD concurs with <br /> both of these recommendations. The work plan for the monitoring well is to be submitted to EHD by April 15, <br /> 2005. <br /> The report of findings for this monitoring well installation is to be formatted in the concept of a Site Conceptual <br /> Model and should be constructed for this site utilizing all available data collected so far. Areas where <br /> delineation interpretations and conclusions are drawn without data to support them should have investigation <br /> proposals of those areas included in the next work plan submitted to EHD. <br /> On March 16,2005, EHD conducted a field inspection during the monitoring and sampling event for the first <br /> quarter 2005. Many discrepancies/violations were noted with the security of each of the monitoring well's traffic <br /> boxes and casings. All boxes were missing bolts and many monitoring well casings were not within the traffic <br /> box,were not surrounded by cement grout, and have an effective watertight locking cap on the end. <br /> You are hereby directed to immediately fix, repair, and provide the following: <br /> • All of the monitoring well traffic boxes are to have bolted lids and, <br /> • All monitoring well traffic boxes are to be set at the level of the surrounding asphalt grade or slightly <br /> higher to avoid receiving surface drainage and, <br /> • The top of all monitoring well casings are to end within the traffic box containment, be completely <br /> surrounded by cement grout, and each casing is to be secured with a locking watertight cap. <br /> EHD will conduct another field inspection during the Second Quarter 2005 monitoring event to evaluate the <br /> wellhead repairs.
