Submit written request for overtime review in order <br /> K to expedite review. Additional fee will be charged for <br /> this review at rate of $52 . 50 per hour with minimum <br /> G,q3 � O one hour charge . <br /> In addition, review of th6 work plan reveals the following <br /> information is lacking: <br /> /1 Location of any drinking water supply wells within 50 <br /> �j C)�1F ft . of the monitoring well locations. * <br /> 2 . Location of any septic systems or sanitary sewer lines <br /> oK within 50 ft . of the proposed well locations.* <br /> 9-� 3�a FF--3 . The well permit application must be signed by the C-57 <br /> ly licensed well driller, or we must have a signed statement <br /> yL from the driller authorizing you or your company to sign <br /> R �3xr O as his authorized agent. <br /> *If neither of these conditions exist, please make that <br /> statement on back of permit application.
