Groundwater Treatment Pilot Plant -3- 15 October 1993 <br /> DDRW, Tracy <br /> out Report (M-{9 as noted above, it is difficult to determine if these excessive flow rates <br /> contributed to the failure of the injection wells. <br /> 4. General Comment; Although pressure readings were taken at the time of sampling in the <br /> injection, extraction, and performance monitoring wells as described in the Monitoring and <br /> Reporting Program of the WDR, no analysis of this data was included in the report. Analysis <br /> of this data should be contained in future reports whether or not unexpected results were <br /> attained. <br /> Three Month Evaluation Report No. 2 <br /> 1. Page 3-8, Table 3-3; The specific conductivity of performance monitoring well LM-62 was <br /> 1,125 µmhos/cm on 22 March 1993. This value is considerably larger than the 25 January <br /> 1993 conductivity of 268 µmhos/cm in this same well. It is the concern of the Board that this <br /> well be monitored closely in the future to determine the significance of this jump. <br /> 2. Page 3-10, Section 3.1.2.2; This section states that monitoring well LM-102 is screened in <br /> both the middle and upper horizons of the formation. However, it is only screened in the <br /> middle horizon and well LM-101 should be substituted in its place as being screened in the <br /> upper horizon. <br /> 3. General Comment; As commented above on the Three Month Evaluation Report No.I (M-49, <br /> the report should give reference to the pressure readings as required by the WDR in tabular or <br /> text format including an analysis. <br /> Operation and Maintenance Manual <br /> 1. Appendix E, Extraction Well Piping; The text indicates that Figure E-5 contains a photo <br /> showing an extraction well vault and old PVC piping as well as a photo showing newly <br /> installed galvanized piping. It also states that Figure E-6 contains extraction well pump <br /> photographs. These two figures are reversed. Figure E-5 contains the extraction well pumps <br /> and E-6 contains the photos of the vault and piping. <br /> 2. Miscellaneous Section, Installation; This sublet states that the flange at the top of the air <br /> stripper was leaking before and after Calcon had attempted to tighten the bolts that brace the <br /> flange. However, there is no discussion as to how the leaking flange was corrected. Was the <br /> problem corrected? How was it accomplished? <br />
