The Discharger shall notify the Board and the Department of Health Services, within 24 hours, of any shutdown of the interim ground water system. This notification shall include the cause of the shutdown and the corrective action taken (or proposed to be taken) to restart the system. 7. The Discharger shall notify the Board and the Department of Health Services, within 24 hours, of any spill associated with the operation of the interim ground water system. This notification shall include the size and cause of the spill , any immediate damage to the environment, and the corrective/cleanup actions taken and/or proposed. 8. The Discharger shall submit to the Board and the Department of Health Services the "Monthly Operation Reports" by the 15th day of the following month. These operational reports should contain a summary of the 0&M activities for that month, including those activities contained in the Preventive Maintenance Schedule, prepared by Ensotech, Inc. (dated 22 March 1990 and any subsequent updates) , and in Discharge Specification C.4. (vi) . In addition, the Monthly Operation Report shall contain a summary of any shutdown and/or spill events that occur that month. 9. The Discharger shall submit to the Board and the Department of Health Services all revisions to the "Preliminary Ground Water Model Report" , dated 24 March 1990. In addition, the Discharger shall provide to both the Board and the Department of Health Services the "Model Operations Manual" for the ground water contaminant transport model , including software, data, and operating instructions. 10. The Discharger shall submit to the Board and the Department of Health Services the "IRM Evaluation Report" and the "System Evaluation Report". The "System Evaluation Report" shall determine if the performance monitoring well network (MW-1 through MW-10 and the nearby RI/FS monitoring wells) provides sufficient hydrologic and water quality data to accomplish the following objectives: a. Monitor water levels and water quality within the permeable zone screened by the extraction wells, and within overlying and underlying zones. b. Verify that the capture zones of individual extraction wells overlap (i.e. , no gaps exist) , and that off-Depot migration of contaminants is being reduced in the vicinity of the extraction field. C. Determine hydrogeologic and water quality parameters of the ground water regime in order to adequately calibrate the ground water contaminant transport model.
