R5-2006-0094, CITY OF <br /> LATHROP, SAN JOAQUIN COUNTY <br /> I have reviewed the 14 November 2006 Monitoring Well Repair Workplan prepared by <br /> Hydrofocus, Inc. The workplan also contains an 11 July 2006 Well Destruction Work Plan <br /> prepared by Erler & Kalinowski, Inc., which describes a procedure to drill out and destroy a <br /> groundwater monitoring well. <br /> The Hydrofocus Inc. workplan describes five wells that have been excavated by construction <br /> activities. The wells are: MWM-1, MWM-2, MWM-10, MWM-18, and MWM-23. Well MWM-18 <br /> has already been properly abandoned. To date, the remaining well casings are believed to be <br /> in good condition with the exception of Well MWM-10, which needs to be destroyed and <br /> replaced. The workplan describes a plan to cut off the remaining well casings, reset the <br /> wellheads, survey the new well casing elevations, destroy Well MWM-10, and replace Well <br /> MWM-10. The Hydrofocus workplan proposes to destroy Well MWM-10 using the procedure <br /> described in the Erler & Kalinowski workplan. The workplan is acceptable, please proceed <br /> with the well repair and destruction of Well MWM-10. However, an acceptable workplan for <br /> replacing Well MWM-10 has not been submitted. That work cannot be performed at this time. <br /> Waste Discharge Requirements Order No. R5-2006-0094 requires submittal of a Master <br /> Groundwater Well Installation and/or Destruction Workplan at least 90-days prior to the first <br /> scheduled construction or destruction of any groundwater monitoring well. The well <br /> destruction described in the workplan is considered urgent to protect groundwater quality. <br /> Installation of the replacement well shall be performed as soon as possible; therefore, by <br /> 15 February 2007, the Discharger shall submit the Master Groundwater Well Installation <br /> and/or Destruction Workplan consistent with the WDRs. <br /> If you have any questions on this information please contact me at (916) 464-4616. <br /> TIMOTHY-R.
