The Addendum responds to the Regional Board's 22 April 2005 Conditional Approval
of Groundwater Monitoring Workplan. The workplan was prepared in response to Waste Discharge
Requirements (WDRs) Order No. R5-2005-0045, Provision G.l.c. The 18 April 2005 Workplan, with
the 26 April 2005 Addendum, is acceptable. Please proceed with the well installation activities.
A number of existing wells are proposed for continued use as groundwater monitoring wells. This may
be acceptable based on well construction and condition. Please evaluate each of the existing wells
proposed for continued use and determine if their construction complies with California Well Standards
Bulletin 74-90. If inadequate information exists to determine that the wells were constructed consistent
with Bulletin 74-90 then they shall be destroyed and replaced with new wells as described in the
Workplan and Addendum described above. The Monitoring Well Installation Report required by WDRs
Order No. R5-2005-0045 Provision G.Ld shall include boring logs and well construction details for all
wells that will be included in the monitoring network. If no well construction detail is available, the well
shall be investigated by video log or other means to determine the well construction.
If you have any questions on this conditional approval or requirements for existing well installation,
please telephone me at(916)464-4616.
TIMOTHY !O'BRiEN
Waste Discharge to Land Unit
cc: Mike Huggins, San Joaquin County Environmental Health Department, Stockton
John Fio, Hydrofocus,Dixon
California Environmental Protection Agency
W sSObnenTSan loaquinU.ethrop WwR Phase NCeamn 12 May OS.doc
Cj Recycled Paper
