It should be noted that staff has <br /> attempted to prepare tentative WDRs despite the incomplete RWD by requesting data and figures from <br /> the Discharger and by addressing the incomplete information items with technical reports that would be <br /> included in the provision section of the WDRs. However, the requested information has been slow in <br /> coming and incomplete,therefore the RWD has been determined to be incomplete and an addendum or <br /> resubmittal is necessary. The following additional information must be submitted in order to allow <br /> completion of the tentative Waste Discharge Requirements (WDRs): <br /> 1. The RWD does not provide information on all the landowners for the land application sites and <br /> wastewater treatment facility. Although the RWD states the facilities will eventually be <br /> transferred to the City of Lathrop, no schedule for such a transfer is provided and staff's request <br /> for that information has not been provided. Each landowner and/or treatment system owner shall <br /> submit a completed Form 200 and will be named as a"Discharger" in the WDRs. Please refer to <br /> the enclosed Form 200 for instructions on completing the form. <br /> 2. Information on the California Environmental Quality Act was not provided in the RWD. Section <br /> V of Form 200 indicates a"Notice of Determination"has been filed but a copy of the document <br /> is not provided. Please provide a copy of the certified CEQA document. The document <br /> provided must include all mitigation measures included as part of adoption of the document. <br /> 3. The California Department of Health Services has requested additional information on the <br /> recycled water system to add detail to the September 2003 Title 22 Engineering Report. Please <br /> provide copies of all correspondence with the DHS. Tentative WDRs cannot be distributed until <br /> the Engineering Report has been finalized and approved by DHS. <br /> 4. Please present the assumptions on the wastewater generation rate and the strength of the <br /> wastewater. Please describe how many residences,businesses, and industrial buildings will be <br /> constructed and the types and quantities of wastewater that is anticipated from the dischargers. <br /> California Environmental Protection Agency <br /> 0 Recycled Paper <br />
