The form was submitted on 28 July 2004; however, it was <br /> signed by Randy Johnson, Project Manager. Signatories to Form 200 must be corporate officers, <br /> partner/proprietors, or executive officer/elected or appointed official. Please refer to the instructions in <br /> Section 8 of the attached Form 200 and resubmit the form. Because there are two owners and a contracted <br /> operator, each entity must submit a completed Form 200. The form is attached for your convenience; it is <br /> also available on the Regional Board's web page at: http://www.swrcb.ca.gov/sbforms/form200.p <br /> The submittal of Form 200 is required by Provision F.13, Waste Discharge Requirements ()YDRs) Order <br /> No. 5-01-251, which requires notification of any ownership change to the Regional Board.
