�•► In KLEINFELDER <br /> ATTACHMENT A <br /> Scope of Work/Workplan <br /> Limited Soil and Groundwater Assessment <br /> Catlin—Crossroads Commerce Center <br /> Lathrop, California <br /> File No.: 20-4445-03.G01 <br /> December 28, 2000 <br /> INTRODUCTION <br /> Kleinfelder is pleased to present this proposal to Mr. Steven Dolim of Catlin Properties. This <br /> proposal was developed based on conversations and correspondence with Mr. Steven Dolim <br /> regarding wastewater pond/spray field infiltration issues at the site. Kleinfelder is committed to <br /> providing quality service to our clients, commensurate with their wants, needs and desired level <br /> of risk. If a portion of this proposal does not meet the needs of Crossroads Commerce Center or <br /> if those needs have changed, Kleinfelder will consider appropriate modifications, subject to the <br /> standards of care to which we adhere as professionals. Modifications such as changes in scope, <br /> methodology, scheduling, and contract terms may result in changes to the risks assumed by <br /> Crossroads Commerce Center as well as adjustments to our fees. <br /> This workplan has been prepared at the request of Crossroads Commerce Center for the <br /> installation of monitoring wells at the subject site to be used in sapling and evaluation by others. <br /> Well locations and design were specified by Gary Litton of the University of the Pacific. <br /> Based upon previous work done in the vicinity by others, groundwater in the vicinity of the site <br /> is anticipated to occur at a depth of 71/2 feet below grade. <br /> SCOPE OF WORK <br /> Task 1 Drilline Permit Submittal and Pre-Mobilization Preparation <br /> Kleinfelder would install five (5) monitoring wells on the site. The San Joaquin County Public <br /> Health Services Department Environmental Health Division (PHS/EHD) requires approval of a <br /> workplan, application for a boring permit, and payment of fees prior to conducting assessment <br /> activities. It is assumed that this workplan will suffice to obtain approval from the PHS/EHD. <br /> Kleinfelder will prepare boring permit applications for submittal to PHS/EHD. The permits and <br /> workplan must be approved prior to conducting any drilling or sampling. <br /> Additionally, PHS/EHD will bill Crossroads Commerce Center for any oversight fees incurred <br /> above the initial 3-hour minimum and permit fee to be submitted by Kleinfelder. These fees will <br /> be billed directly to Crossroads Commerce Center by the PHS/EHD. Kleinfelder will forward <br /> the appropriate "billing and compliance acknowledgement forms" to Crossroads Commerce <br /> Center for signature.
