Attached you will find the <br /> "Notice of Reimbursement" and the "Site Letter" notifying you of your rights and obligations under <br /> Federal and State laws. Since some time has past since the contamination was first evidenced at <br /> the site, PHS/EHD has summarized the site history for your consideration and has provided <br /> regulatory directives for your response. <br /> Tank #39-1327-01 Removal <br /> On April 21 , 1988, the 350 gallon gasoline tank was removed and one soil samples collected <br /> beneath the tank. Piping associated with the operation of the tank has not been removed. PHS/EHD correspondence dated March 4, <br /> 1994, stated that further investigation of the soil and/or groundwater contamination was required. <br /> PHS/EHD has enclosed an additional UST Clean-up Fund Application and encourages you to submit the <br /> completed application to the State Water Resources Control Board as soon as possible. <br /> In order to initiate the investigation, please submit by December 2, 1996 a work plan to obtain additional soil <br /> and/or groundwater information. <br /> PHS/EHD endeavors to work cooperatively with tank owners, and hopes that the extent of soil and/or <br /> groundwater contamination is limited so that a speedy and inexpensive site certification can be obtained.
