(Newark-Sierra), Harding Lawson Associates (HLA) submits
this boring permit application and site specific Health and Safety Plan for a limited phase II investigation
at the above referenced site. HLA's Work Plan, Courtyard Tanks and Sump, Newark Paperboard Mills, 800
West Church Street, Stockton California was submitted to the San Joaquin County Public Health Services,
Environmental Health Division (PHS/EHD) in response to the PHS/EHD letter dated September 30, 1997
requiring a work plan for additional investigation at the above site (Plate 1). HLA's work plan was
approved by PHS/EHD in a letter to Newark-Sierra dated January 5, 1998.
Specifically, the application is for two geoprobe-style soil borings to depths of approximately 25 feet bgs
at locations shown on the attached plate. If groundwater is encountered in the boring closest to the
former sump (Plate 2) a hydropunch or temporary well point sample will be collected. Enclosed are a
check for$89.00 covering the permit fee, a completed permit application with an attached site plan, a
copy of the Well Drillers License with expiration date, a copy of the Workers Compensation certificate of
coverage, and a copy of the well driller's authorization letter to sign permits.
The field work is tentatively scheduled for the month of February, with Gregg Drilling and Testing Inc., a
C-57-licensed (#485165) well driller from Martinez, California. The work will commence at about 8 AM
and will probably take one day to complete. Grout inspections will be scheduled at least 48 hours prior to
the requested inspection date. Your ongoing cooperation on this project is greatly appreciated by HLA
and Newark-Sierra. If there are any questions, please do not hesitate to call.
Yours very truly,
HARDING LAWSON ASSOCIATES
 4 aazmc—
Gary Haeck, Ph.D., R.G.
Project Geologist
Enclosures: Site Safety Plan
Plates 1 and 2
Completed Permit Application with Boring Locations
Check for$89.00 in permit fees
Well Drillers License
Well Drillers Authorization Letter
Drillers Workers Compensation Certificate
GDF/GLF/tb/G:\WP\GDH\GH40344I.332
Engineering and
Environmental Services 10265 Rockingham Drive,Suite 150,Sacramento,CA 95827 916/364-0793 Fax:916/364-5633
