RECEIVE® <br /> MAR 2 7 2015 <br /> ENTAL <br /> Dear San Joaquin County Environmental Health Department, ENV PERMITISERVICESLTH <br /> This is response to your Hazardous Waste Inspector Audit performed at the Big Lots! Store located at <br /> 380 S. Cherokee Lane in Lodi, Ca on 12-29-14. The following are the violations noted and our response <br /> to them: <br /> 102—1. Leaking bottle of dish soap seen at register was double bagged with Spill Magic and placed in <br /> the Toxics bin. <br /> 2.The 80 bottles of expired Listerine were removed from the shelf and placed in the Flammables <br /> bin. <br /> 3. The 3 cracked containers of Good & Clean wet wipes were removed from the clearance section <br /> and placed in the Corrosive Base container. <br /> 104—The Emergency Contact list has been updated. The Store Manager is listed first,then the District <br /> Manager is second with phone numbers. Copies of the list are posted at the Haz Mat station and at <br /> every phone in the store. <br /> 105—Employee training on handling hazardous materials—The company initiated an electronic based <br /> training for all employees in California. All employees at this location were trained during the month of <br /> February and March 2015. Attached is a list of the employees trained and the dates of training. <br /> 202—The company started the Hazardous Waste material handling program in late 2013. Since then we <br /> have been working to ensure compliance of the program. <br /> 405—Leaking bottle of Bleach in wrong container was placed in correct container. Re-training of the <br /> management team on the correct types of materials went in what bins happened immediately after the <br /> inspection and as mentioned above, all employees at the store was completed in February 2015. 1 have <br /> inspected and quizzed numerous employees at this location on their knowledge of the program. <br /> 601—Aerosol can of glass cleaner observed in the"product withdrawals" area was removed and placed <br /> in the Flammable bin. <br /> 605—Totes at the accumulation station have all been labeled correctly and training of the management <br /> team on how to correctly label the totes was completed immediately after the inspection. <br />
