In summary, all items have been <br /> corrected as of December 5, 2009. Each item noted on the report has been specifically addressed <br /> below illustrating the corrective action and return to compliance: <br /> • Line item#12—"Facility not maintained to minimize the release of a hazardous waste." <br /> The oil absorbent located next to a piece of equipment was immediately cleaned-up and this <br /> small amount of material added to our hazardous waste container to be properly disposed of <br /> by a certified/licensed contractor. <br /> • Line item#20—"Failed to properly label containers of hazardous waste." <br /> The labels applied to used oil, aerosol cans and used absorbent hazardous waste collection <br /> containers are now completely filled out—specifically, the physical state and hazardous <br /> properties have been identified. <br /> • Line item#88—"Modified emergency coordinator information lacking or insufficient." <br /> The emergency contact list posted by emergency use phones and plans has bee reviewed for <br /> accuracy and completeness. A copy of is attached to this letter as requested. <br /> A completed "Return To Compliance Certification" form is being provided with this letter. It is my <br /> understanding that the information noted above will satisfy the"Statement"requirement of this <br /> form. <br /> Please be aware that compliance with all agency rules and regulations is of paramount importance <br /> to us. The findings of this report have been thoroughly discussed internally with all staff. Our goal <br /> is to be in compliance at all times.
