AN Environmental Health Department <br /> QU <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> THE GRADUATE 2207 COUNTRY CLUB BLVD, STOCKTON TMay 24, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to complete and/or submit hazardous materials inventory for all reportable materials on site. <br /> The Hazardous Materials Inventory forms for carbon dioxide and nitrogen are not complete. The size of the largest <br /> container, average daily amount, days on site, storage pressure, and storage temperature elements have not been <br /> completed. This information must be current and submitted as part of the business plan. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, enter the correct or updated <br /> information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not address loading areas, internal roads, access and exit points (from street). A site map shall <br /> contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit <br /> points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and <br /> emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System (CERS) annually by January 15. The hazardous materials inventory shall be submitted by January 15 of <br /> each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br /> CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years. Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 12 HSC 25505(c) Failed to have the business plan readily available to facility or EHD personnel. <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request. The emergency <br /> response plans and procedures, the inventory of information required by this article, and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section.
