Manifest numbers 002121771FLE (12/16/09) and 002143284FLE (10/3/10) were not found <br /> on site. Hazardous waste generators shall retain copies of all manifests for three years or until a copy <br /> signed off by the destination facility is received. The signed off copy shall be retained and made readily <br /> available for review for three years. If the generator did not receive a copy of the manifest with the <br /> handwritten signature of the owner or operator of the facility to which the generator's waste was <br /> submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br /> shall contact the transporter and/or the owner or operator of the designated facility to determine the <br /> status of the waste. If the generator did not receive a copy of the manifest with the handwritten <br /> signature of the owner or operator of the facility to which the generator's waste was submitted within 60 <br /> days of the date the waste was accepted by the initial transporter, the generator shall submit a legible <br /> copy of the missing manifest, with some indication that the generator has not received confirmation of <br /> delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to <br /> DTSC. By June 9, 2011, submit to the EHD a copy of the signed off copy of the manifests or a copy of <br /> the information submitted to DTSC. <br /> REPEAT VIOLATION <br /> Notice to Comply - Minor violations: (52, 85) <br /> 52. At the time of inspection, it could not be demonstrated (not necessarily documented) that employees <br /> who handle hazardous waste were properly trained. According to Mr. Howell, only the UST designated <br /> operator training is conducted. An emergency coordinator and modified contingency plan information is <br /> lacking. The generator must ensure that all employees who handle hazardous waste are thoroughly <br /> familiar with proper waste handling and emergency procedures. There must be at least one emergency <br /> coordinator on site or on call to coordinate emergency response measures, and the following information <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY W AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector.
