The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training . This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years . Immediately <br /> provide employee training for appropriate personnel and submit a copy of the training records to the EHD . <br /> This is a repeat violation , Class II . <br /> 12 HSC 25505 ( c) Failed to have the business plan readily available to facility or EHD personnel . <br /> Facility could not provide the Emergency Response Plan and the Chemical inventory upon request . The emergency <br /> response plans and procedures , the inventory of information required by this article , and the site map required by <br /> this section shall be readily available to personnel of the business or the unified program facility with responsibilities <br /> for emergency response or training pursuant to this section . Immediately make arrangements to have the business <br /> plan readily available for review . <br /> This is a Class II violation . <br /> Overall Inspection Comments . <br /> An inspection report was provided to the facility operator on the day of inspection , June 5 , 2019 , On June 28 , <br /> 2019 , the report was amended by the EHD . This amended report replaces the initial report . <br /> The report was amended to delete violation 8 and add violation 7 . <br /> Facility does not have internal roads , emergency shutoffs and these elements were not addressed on the site <br /> map . Please address this by either writing " NA" for not applicable or a statement that says the element is not on <br /> site . Please correct this by the next submittal . <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation , and any supporting <br /> paperwork , within 30 days of receiving the completed inspection report . <br /> Starting September 1 , 2018 , all in - office CERS help will be provided at EHD hourly rate ( $ 152 ) .
