The date that the tank was <br /> permanently closed is missing on the tank. When a tank is not in use, it must be permanently closed by meeting the <br /> following conditions: <br /> -remove all liquid and sludge from each container and connecting line <br /> -disconnect and blank off all connecting lines and piping have from the tank <br /> -close and lock all valves <br /> -post a sign conspicuously stating that it is a permanently closed container and denoting the date of closure. <br /> Immediately'Permanently Close"all tanks that are not being used or ensure that the Spill Prevention, Control, and <br /> Countermeasure Plan includes all petroleum tanks that have not been permanently closed. <br /> This is a repeat violation, Class II. <br /> 609 CFR 112.7(a)(3vi), 112.7(a4) Plan failed to adequately contain procedures for reporting a discharge. <br /> The Spill Prevention, Control, and Countermeasure(SPCC)Plan does not provide information and procedures to <br /> enable a person to adequately report a discharge. A facility response coordinator is not indicated on the emergency <br /> phone list on page 14 of the SPCC plan. If a response plan was not submitted to the Regional Administrator,this <br /> information must be included in the SPCC Plan. Immediately ensure that the SPCC Plan includes this information <br /> and submit a copy of the revision to the EHD. <br /> This is a minor violation. <br /> 622 CFR 112.7(f)(3) Failure to conduct complete annual discharge prevention briefings for oil-handling personnel. <br /> Discharge prevention briefings are not scheduled at least once a year. The last record for discharge prevention <br /> briefings was from 11/1/2017. Discharge prevention briefings for oil handling personnel must be scheduled and <br /> conducted at least once a year to assure adequate understanding of the SPCC Plan for that facility. Such briefings <br /> must highlight and describe known discharges or failures, malfunctioning components, and any recently developed <br /> precautionary measures.
