This work was <br /> performed on 27 March 1996 according to the attached Well Destruction Permit and was <br /> coordinated by Ms. Margaret Lagorio and supervised by Mr. Steven Sasson of the San Joaquin <br /> County Public Health Services Environmental Health Division. <br /> The well boxes enclosing wells MW-1 through MW-3 were excavated and removed and <br /> approximately one-foot of the casing was cut before well abandonment activities were initiated. <br /> Approximately` 10 gallons of grout were poured nto'each well followed by the placement of <br /> a well cap with an attached pressure valve. Each well cap fitting was then filled with air <br /> supplied from an air compressor at 125 p.s.i. for approximately five minutes or until air was <br /> observed escaping from the surface. After pressurizing each well, the wells were then capped- <br /> off with an additional 5 gallons of grout or until the grout reached the top of casing. <br /> Approximately six (6) feet of well casing from MW-4 was inadvertently removed during the <br /> attempted removal of the well box.
