Accordingly, the Estate of Frederic Clarke <br /> wishes to properly destroy all three monitoring wis(MW-1 through MW-3)remaining at the site. <br /> Enclosed please find a permit application and ppropriate fees to destroy these wells. Well <br /> destruction methods are described below. <br /> i <br /> All work will be conducted in accordance with a LUFT Field Manual, the Tri-Regional Board <br /> Staff Recommendations, Article 11 Corrective ction Regulations of Chapter 16 Underground <br /> Storage Tanks Regulations, and California Well S andards Bulletin 74-81 and 74-90. <br /> Prior to drilling,Underground Service Alert will b requested to identify utilities leading to the site. <br /> All field personnel on-site will review and sig the Site Health and Safety Plan, prepared in <br /> accordance with OSHA 1910.120, at the start of d Le field day. <br /> Wells MW-1 through MW-3 will be destroyed b5 over-drilling. These wells are constructed with <br /> 2-inch diameter PVC well materials within 8 inch diameter boreholes to a total depth of <br /> approximately 45.5 feet below ground surface (b s). Prior to drilling, depth to bottom of all wells <br /> will be verified using a weighted tape. Under upervision of a Clearwater geologist, Mitchell <br /> Drilling Environmental Corporation will use a CD 1E 75 high-torque drill-rig equipped with 8-inch <br /> diameter hollow-stem augers to over-drill each we ll. The augers will be advanced at least one-foot <br /> beyond the total depth of each well to ensure all ell materials are removed from the boring. The <br /> geologist will note materials removed from a ch boring.
