The State Water Resource Control Board(SWRCB)will invoice you directly for activities performed <br /> under the auspices of the Local UST Oversight Program. The goal of the site evaluation performed by Public <br /> Health Services Environmental Health Division(PHS/EHD) is to certify closures of underground tank leakage sites. <br /> Closure certification occurs only after mitigation activities are completed. <br /> In addition, the "Regional Board Staff Recommendations For Initial Evaluation and Investigation of Underground <br /> Tanks" and "Appendix A" reporting format and the LUFT Manual shall be utilized as guidance documents in the <br /> preparation of work plans and/or reports. <br /> At your request PHS/EHD sent you a letter dated September 20, 1995 indicating the compliance status of the UST <br /> at the time of removal, with a copy of UAR 93-U16, for purposes of applying to the SWRCB for reimbursement. <br /> In addition, PHS/EHD has recieved, reviewed and approved the workplan dated August 7, 1995 prepared by <br /> Upgradient Environmental Consultants for this site.
