The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Evil Motosport as of June 18, 2019. <br /> Open violations from April 19, 2019 inspection <br /> Violation #101 - Managed a hazardous waste without an identification number. <br /> This facility's hazardous waste ID number is inactive. A hazardous waste generator shall not treat, store, dispose <br /> of, transport or offer for transportation, hazardous waste without a hazardous waste ID number. Go to the DTSC <br /> web site http://www.dtsc.ca.gov/IDManifest/PERMHWID.cfm for the form to reactivate the ID number . <br /> Immediately reactivate your EPA ID number and submit evidence to the EHD. <br /> Please reactivate your EPA ID number. Send notice to EHD when reactivated. <br /> Violation#110- Failed to keep signed copy of manifests from the designated facility for three years. <br /> Copies of uniform manifests for the disposal of hazardous waste for 2016 were not found on site. Hazardous waste <br /> generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br /> consolidated manifesting procedure on site for three years and have them readily available for review. Immediately <br /> locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the <br /> EHD. <br /> Violation #113- Failed to keep copies of consolidated manifesting receipts for three years. <br /> Copies of consolidated manifests for the disposal of hazardous waste for 2016 were not found on site. Hazardous <br /> waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts used in a <br /> consolidated manifesting procedure on site for three years and have them readily available for review. Immediately <br /> locate a copy of all manifests and receipts for the last three years, maintain them on site, and submit copies to the <br /> EHD. <br /> Violation #301 -Facility not maintained to minimize the release of a hazardous waste. <br /> Used oil observed leaking from car being maintenance at the front of the shop and hazardous waste storage area <br /> observed to have used oil in secondary containment and surrounding area. Facilities shall be maintained and <br /> operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water <br /> which could threaten human health or the environment. Immediately clean stains and leaks , and manage <br /> according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining <br /> how this waste was managed. <br /> Please clean hazardous waste and properly dispose of them. Provide disposal receipts to EHD. <br /> Violation #304- Failed to maintain adequate aisle space for unobstructed response to emergencies. <br /> Hazardous waste storage area is blocked by truck. Adequate aisle space shall be maintained to allow the <br /> unobstructed movement of personnel, fire protection, spill control equipment, and decontamination equipment to all <br /> areas of facility operation in an emergency. Immediately maintain aisle space to allow the unobstructed movement <br /> of personnel, fire protection, spill control equipment, and decontamination equipment to any area of facility operation <br /> in an emergency. <br /> Please move truck to another spot and allow aisle space for storage area. <br /> Submit statement to EHD. <br /> Page 1 of 1 <br />
