The days on <br /> site element has not been completed and the storage pressure is wrongly identified as"ambient' vs. "above <br /> ambient'for the compressed oxygen; the Federal Hazard Categories have not been updated for the motor oil and <br /> compressed oxygen. This information must be current, accurate, and submitted as part of the business plan. <br /> Immediately log into the California Environmental Reporting System(CERS) at http://cers.calepa.ca.gov/, enter the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to complete and/or submit a site map with all required content. <br /> The map does not show/address internal roads, access and exit points (all access and exits for the building), <br /> emergency shutoffs, hazardous material handling and storage areas(motor oil), and emergency response <br /> equipment(absorbent). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, <br /> storm and drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material <br /> handling and storage areas, and emergency response equipment. If a site map element is not applicable for your <br /> facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.Ga.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class Il. <br /> 9 HSC 25508(a)(1) Failed to annually review and certify that the business plan is complete, accurate, and up to date. <br /> The business plan information has not been reviewed and resubmitted in the California Environmental Reporting <br /> System(CERS) annually for 2018 and 2019. The hazardous materials inventory shall be submitted by January 15 <br /> of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the <br /> CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 11 HSC 25505(a)(4) Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> Employee training records were not available at the time of inspection. The business plan shall include provisions <br /> for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be <br /> documented electronically or by hard copy and shall be made available for a minimum of three years.
