The NFA closure letter may not be issued without this documentation. LOP Staff directs RP to destroy all MWs, remove all waste material & provide documentation of proper waste disposal. LOP staff reviews and approves work plan for well destruction. MW Destruction permits issued (after 60 days of PPN) & Johnny notified of impending work. `T LOP Staff documents destruction of all MW. GT "Groundwater Monitoring" screen updated to indicate wells are destroyed. The NFA closure letter may not be issued until MWs are destroyed.(1/u(J' LOP Staff verifies and flags documentation received indicating all onsite wastd material removed and properly disposed and verified with EHD accounting all SR& D site fees have been paid. The NFA closure letter may not be issued without this documentation. GT"Closure Review" screen marked "Yes" at"Is This Case Ready for Closure?" LOP Staff asks clerical to prepare NFA& CCS for Director's signature within 30 days of MW destroyed and waste material properly disposed, & provides RPs name and addresses to be sent original NFA. PG/PC reviews file with flagged documents and approves issuance of NFA letter by Director. Signed CCS and NFA are copied. Originals mailed to RPS. Copies filed into LOP File, PC, and staff to upload into GT. Clerical staff updates Envision to reflect site is Closed. LOP staff pulls all site files for clerical staff to file in LOP Closed File cabinets, until scanned into Questys. LOP staff updates GT"Project Information" screens to reflect site status as Closed.
