The form you used is for domestic-public <br /> water well installations and pump repairs and cannot be used for CPT work at contaminated UST sites. <br /> Attached are the correct forms for this type of work..Please save these forms and use them for ALL boring proposals/work <br /> at UST/LOP and former UST sites. Do NOT use the "Well / Pump Permit"form for work at UST sites. Unless you <br /> (Cambria, Shell) plan on installing, repairing, removing, DOMESTIC, Public Drinking water wells, this form should be <br /> discarded. <br /> Please complete and send the correct form to EHD. The check is still valid and I will hold it until the correct form is <br /> submitted. You may fax it to me if you like, at(209) 468-3433, or send it by US Mail...your choice. <br /> also attached is a UNIT IV"LCD"form that is simpler and typically used with UST/LOP well permit applications. I <br /> recommend you save this form also and use it with the UNIT IV"Well Permit Application Form" that you may submit in the <br /> future. <br /> If at all possible, I request that you advise others at Cambria that you have the correct forms for UST/LOP borings and well <br /> installations in San Joaquin County and make them available. Mr. Neely and Ms. Cool have in the past, and continue to <br /> use these forms, althought at times not the most current update.. Please note the date at the lower right corner. The latest <br /> form with the most current update is"9/30/02". Previously dated forms are obsolete and should be discarded.
