I received your CPT boring application and check#1740 for work at 1267 Country Club Blvd, Stockton. You have used a form not accepted for'contam inated-LOP/U ST' site work. The form you used is for domestic-public water well installations and pump repairs and cannot be used for CPT work at contaminated UST sites. Attached are the correct forms for this type of work..Please save these forms and use them for ALL boring proposals/work at UST/LOP and former UST sites. Do NOT use the "Well / Pump Permit"form for work at UST sites. Unless you (Cambria, Shell) plan on installing, repairing, removing, DOMESTIC, Public Drinking water wells, this form should be discarded. Please complete and send the correct form to EHD. The check is still valid and I will hold it until the correct form is submitted. You may fax it to me if you like, at(209) 468-3433, or send it by US Mail...your choice. also attached is a UNIT IV"LCD"form that is simpler and typically used with UST/LOP well permit applications. I recommend you save this form also and use it with the UNIT IV"Well Permit Application Form" that you may submit in the future. If at all possible, I request that you advise others at Cambria that you have the correct forms for UST/LOP borings and well installations in San Joaquin County and make them available. Mr. Neely and Ms. Cool have in the past, and continue to use these forms, althought at times not the most current update.. Please note the date at the lower right corner. The latest form with the most current update is"9/30/02". Previously dated forms are obsolete and should be discarded. This latest update incorporates "ozone", "GP/CPT", and "multi-level"wells.
