Offsite
access agreements are required for private properties and City of Stockton Encroachment permits are required
for public right of way access. Separate PHS-EHD boring permit applications and the $89 fee are required for
each parcel where borings are to be placed.
A workplan. including the boring permit applications and access agreements, that proposes multiple well
placement in areas that will aid in determining the lateral and vertical limits of the soil and groundwater plumes
should be submitted to PHS-EHD by October 1, 1997.
Enclosed please find the latest testing and laboratory analyses requirements for gasoline additives/oxygenates
as per the Central Valley Regional Water Quality Control Board. This requirement is to be incorporated in all
soil and groundwater analyses for this site until directed otherwise by PHS-EHD.
You may contact Michael Infurna at(209)468-3454 if you have any questions or wish to discuss the site.
Donna Heran, RENS, Director
Environmental Health Division
Michael Infurna Jr., Senior REHS Margaret , REHS
LOP/Site Mitigation Unit IV Supervisor
MI/
enclosure
C' Enviros- Diane M Lundquist, PO Box 259, Sonoma, 95476-0259 (w/enclosure)
c: Bank of the West- Penny Jasper, 1450 Treat Blvd., Walnut Creek, 94596
c: SWRCB - Martin Clark, PO Box 944212, Sacramento, 94244-2120
c: CVRWQCB - Elizabeth Thayer
A Division of San foaauin Countv Health Care Services
