In order to evaluate whether or not
remediation will be required at this site, San Joaquin County Public Health Services, Environmental Health
Division (PHS-EHD) will require further investigation to be conducted at the site.
A work plan should be submitted to PHS-EHD that will address both the verification of the underground
tank(s) removal and a subsurface investigation that will attempt to characterize the extent of the soil and
groundwater plume still in place.
Boring permits are issued after the work plan is approved and are valid only for the parcel the boring or
well will be on. Off site parcels will require separate PHS-EHD permits, access agreements, and
application fees of$89. Borings into the County or City right of way will require encroachment permits
issued by the agency with jurisdiction over the parcel. Phone numbers for these agencies can be
obtained from PHS-EHD.
Your work plan is due at PHS-EHD no later that October 28, 1996- Please contact Michael Infurna at
(209) 468-3454 if you have any questions.
Donna Heran, REHS, Director
Environmental Health Division
Michael Infurna, Senior REHS Margare agonftR`
EHS
LOP/Site Mitigation Unit Lead Senior
MI/
c: Phillip Metzger- Bank of the West, 1405 Treat Blvd, Walnut Creek, 94596
A Division of San Joaquin County Health Care Sen-ices
