Wilmington Avenue <br /> Carson, CA 90810 <br /> Subject: Bank of the West <br /> 1267 Country Club Blvd <br /> Stockton, CA 95205 <br /> The San Joaquin County Environmental Health Department (EHD) has received and reviewed <br /> Well Destruction Work Plan, dated 6 August 2014, prepared and submitted by your consulting <br /> firm, Conestoga-Rovers & Associates (CRA), on your behalf for the above-referenced site. <br /> In Well Destruction Work Plan, CRA proposed destruction of a total of 19 wells, (16 monitoring <br /> wells and 3 soil vapor probe). CRA will destroy these wells by over drilling groundwater <br /> monitoring wells S-1 through S-8, S-10 through S-17 using 8" or 11" diameter augers and destroy <br /> Soil Vapor Probes VP-1 through VP-3 using an air knife and or hand auger. In the work plan, <br /> your consulting firm, CRA has proposed to submit a brief report documenting the activities to EHD <br /> and a Well Destruction Report form to the Department of Water Resources after all wells have <br /> been destroyed. In addition to the data, include a signed manifest of all water/soil waste related <br /> to the destruction of all the wells at this site. <br /> CRA has proposed to use neat cement as the sealing material for the wells. All water used for the <br /> purpose of sealing a well must be potable water and not water that has been displaced from a <br /> well that is to be destroyed. With these conditions, the EHD approves the work plan as submitted. <br /> Once the public participation period (August 29, 2014 — October 29, 2014) has ended and all <br /> public concerns have been addressed, please submit a completed Master File Record form, a <br /> Well and Boring Permit Application form, an encroachment permit issued by the City of Stockton <br /> with appropriate fees. The permit fee for the destruction of each well is $130, along with a $390 <br /> Inspection Activity fee per parcel. A review of the site records indicate that 16 monitoring wells <br /> and 3 vapor soil vapor probes are currently installed at this site. A permit/inspection fee of <br /> $3,250.00 (130 x 19 + 390 x 2) will be required for the well destruction permits. Notify the EHD <br /> for inspection of all destruction of wells at least two weeks prior to destruction activities. The EHD <br /> will issue a closure letter once and all wells have been properly destroyed and waste manifests <br /> have been submitted to this agency. <br />
