This update was to be included in the next quarterly <br /> report submitted to PHS-EHD. <br /> On February 27, 1996 the Fourth Quarter, 1995 Monitoring Report <br /> was received and the site map was not updated. Additionally, <br /> required items that need to be included in every quarterly report <br /> have not been included. Recommendations, conclusions, and <br /> statements concerning deviations from quarterly requirements were <br /> some of the missing items . <br /> MW-5 was not sampled and no explanation was offered. Also missing <br /> was a notation explaining whether or not the other well head and <br /> traffic box repairs have been made to the remaining wells . <br /> Pursuant to Section 2652 (d) of the Underground Tank Regulations <br /> (California Code of Regulations, Title 23, Subchapter 16) you are <br /> hereby directed to submit a quarterly monitoring report every <br /> quarter to the Environmental Health Division (PHS-EHD) and the <br /> Central Valley Regional Water Quality Control Board (CVRWQCB) in <br /> a timely manner until the investigation and cleanup of the site <br /> is deemed adequate. The minimum requirements are the: <br /> - status of the investigation and cleanup activities <br /> including the results of all investigations completed <br /> to date; <br /> - proposed, continuing, or next phase of investigation; <br /> - method of cleanup proposed or implemented and a time <br /> table or schedule for the completion of the procedure; <br /> - method and location of disposal of the released <br /> hazardous substance and any contaminated soil or <br /> groundwater; <br /> A Division of San Joaquin Countv Health Care Services <br />
