In accordanc,:� with Section 1(h) of the "Standard Conditions Applicable to <br /> Revocable Permits," the permittee shall provide and keep in force during the term <br /> of the permit a policy of public liability insurance against liability for injuries to <br /> persons or property arising out of accidents attributable to the structure on City <br /> property, in amounts to be determined by the Risk Manager. The policy of <br /> insurance so provided shall contain a contractual liability endorsement covering the <br /> liability assumed by the Permittee by the terms of his permit and shall contain a <br /> provision that such policy may not be canceled except after thirty (30) days notice <br /> in writing to be given to the Risk Manager. The copies of these policies or <br /> certificates evidencing the same shall be filed with the Risk Manager of the City <br /> after approval of the City Attorney. <br /> 5. The issuance of this permit by the City does not create any agency on the part of <br /> the permittee, and the permittee will hold the City of Stockton harmless for any <br /> action or damages arising out of the permittee's activities performed under this <br /> permit. <br /> 6. Permittee, or his contractor, shall obtain an Encroachment Permit for the installation <br /> of the well(s). <br /> 7. The well head(s) shall be covered with a meter box or valve box as approved by the <br /> City of Stockton. The top of the box shall be flush with the surrounding ground or <br /> pavement and shall be brought to any future grade adjustments by the permittee at <br /> their own cost. <br /> 8. The provisions of the State of California, Department of Water Resources Bulletin <br /> 74-81 entitled "Water Well Standards: State of California" shall be applicable to this <br /> permit. <br /> 9. The drilling location(s), method, and well design are to be approved by the California <br /> Department of Health Services and the Central Valley Regional Water Quality <br /> Control Board. <br /> 10. Drilling operations that create loud noise shall be confined between the hours of 7 <br /> a.m. and 6 p.m. <br /> 11. Portable mud tanks are to be used. <br /> 12. Disposal of water from the well development shall be in accordance with current <br />
