Use these forms
as "Masters"and reproduce them as you need. The Licensed Contractor Declaration (LCD)can only be signed by the
California State Contractor(C-57 license holder or office representative on file with us), but you may fax a blank form to
them if they don't have it already. If you, as the consultant, signs the application, the C-57 MUST authorize your signature
by signing for you at the bottom of the LCD when they fax it to us. If you have any questions you may call me at(209)468-
3454.
Additionally, for the TOSCO-BP site#11192 at 1403 Country Club Blvd-Stockton, the soil and groundwater analysis for
Lead (Pb)is now NOT required as listed in the work plan approval letter signed by me, dated July 16, 2001. This is the
ONLY constituent deleted and the only modification to the approval for this phase.
hope this answers your concerns..
The appointment to install the four monitoring wells approved this phase is set for Wednesday August 8, 2001 at 8:00 am.
Go ahead and start without me. I will be there as soon as I clear the office that morning.
# E An
WELL-PERMIT-APPL. LCD-contractor
doc declaration+au...
