Marc <br /> Shaping the Future <br /> 1.0 INTRODUCTION <br /> On behalf of Phillips 66 Company (P66), Cardno ATC is submitting this Well Destruction Work Plan (Work Plan) to <br /> the San Joaquin County Environmental Health Department (SJCEHD) for the destruction of 12 monitoring wells, 2 <br /> vapor extraction wells, and 10 ozone sparge wells at the 76 (Former BP) Service Station No.11192, located at 1403 <br /> West Country Club Boulevard in Stockton, California, (Site) shown on Figure 1. This Work Plan was prepared in <br /> response to the Well Destruction Request letter of SJCEHD, dated September 5, 2013. <br /> 2.0 SITE HISTORY <br /> In 1976, two 10,000-gallon USTs containing unleaded gasoline, one 10,000-gallon USTs containing leaded <br /> gasoline, three 3,000-gallon USTs containing motor oil, and one 3,000-gallon USTs containing waste oil, the <br /> associated piping and dispensers, and other facility structures were installed at the Site. The was initially operated <br /> as a gasoline service station by Mobil Oil Company. In May 1989, the Site was purchased by British Petroleum <br /> (BP). The Site was operated under BP ownership until Tosco Corporation (now Phillips 66) assumed operations in <br /> 1994. <br /> 3.0 SCOPE OF WORK <br /> In general, the Scope of Work involves the proper destruction/abandonment of the monitoring wells, extraction <br /> wells (vapor and groundwater), injection wells (ozone and air) present at the Site which have been rendered <br /> obsolete as a result of the Site's regulatory closure. The workplan also includes the activities in support of the well <br /> destruction. The locations of the groundwater monitoring and remediation well networks are shown on Figure 2. All <br /> proposed work will be performed under the direction of a California Registered Professional Engineer or Geologist. <br /> 3.1 Permitting <br /> In advance of the well destruction activities at the Site, Cardno ATC will obtain all necessary permits from the <br /> SCJEHD. In addition, Cardno ATC also will obtain an encroachment permit from the City of Stockton for the wells <br /> constructed within the public right-of-way on Country Club Boulevard (MW-8)and Elmwood Avenue (MW-9 and <br /> MW-10). <br /> 3.2 Site Health and Safety Plan <br /> As required by the Occupational Safety and Health Administration (OSHA) Standard "Hazardous Waste Operations <br /> and Emergency Response" guidelines (29 CFR 1910.120), and by California Occupational Safety and Health <br /> Administration (Cal-OSHA) "Hazardous Waste Operations and Emergency Response" guidelines (CCR Title 8, <br /> Section 5192), Cardno ATC will update the existing Site-Specific Health and Safety Plan (HASP) prior to the <br /> commencement of fieldwork to include the current scope of work. The Site-Specific HASP will be reviewed by field <br /> staff and contractors before beginning field operations at the Site. <br /> 3.3 Subsurface Utility Clearance <br /> As required by California law, prior to commencement of field activities Cardno ATC personnel will conduct a Site <br /> walk and mark the surface of the Site in preparation for identification of subsurface utility/facilities. Cardno ATC will <br /> then submit this information to Underground Service Alert of Northern California and Nevada (USA North), so that <br /> utilities will visit the Site and mark the location of their subsurface facilities/utilities. <br /> Cardno ATC will also request that the Site operator provide any available subsurface utility maps for the Site and <br /> will employ a private utility locator to confirm the locations of potential subsurface utilities present in the proposed <br /> work area. <br /> In addition, each well location will be air knifed to a depth of five feet below ground surface (bgs) to locate and <br /> protect subsurface utility/facilities. <br /> Australia • Belgium • Canada • Ecuador • Germany • Indonesia • Italy • Kenya <br /> New Zealand • Papua New Guinea • Peru • Tanzania • United Arab Emirates <br /> United Kingdom • United States • Operations in 85 countries <br />
