'i <br /> Ellsaesser responded that those wells could be pressure grouted with a minimum three foot <br /> mushroom cap according to San Joaquin County Well Standards. <br /> Wells MW4, MW6, MW9 through MW13, SP-A through SP-F, SPH, SP-J, VW-1 and VW2 were <br /> overdrilled and backfilled with a grout slurry mix as planned. Wells MW-1, MW-3, MW-5, MW-7, <br /> MW-8, SP-G, and SP-K were pressure grouted and then excavated to a minimum of 3 feet bgs, <br /> the well casing cut to a minimum of 2.5 feet bgs and backfilled to 2 feet bgs with neat cement <br /> grout. <br /> For all wells pressure grouted, the grout slurry was placed under 25 pounds per square inch of <br /> pressure for five minutes. <br /> 4.5 Waste Disposal <br /> Soil cuttings and concrete debris generated by the well destruction activities were temporarily <br /> stored in Department of Transportation approved drums until profiling was complete. The waste <br /> was removed by American Integrated Services, Inc. Manifests associated with the drums are <br /> included in Attachment 3. <br /> 5.0 LIMITATIONS AND CERTIFICATIONS <br /> This report was prepared in accordance with the scope of work outlined in Cardno ATC's <br /> contract and with generally accepted professional engineering and environmental consulting <br /> practices iexisting at the time this report was prepared and applicable to the location of the site. <br /> It was prepared for the exclusive use of Phillips 66, for the express purpose stated above. Any <br /> reuse of this report for a different purpose or by others not identified above shall be at the user's j <br /> sole risk without liability to Cardno ATC. To the extent that this report is based on information <br /> provided to Cardno ATC by third parties, Cardno ATC may have made efforts to verify this third <br /> party information, but Cardno ATC cannot guarantee the completeness or accuracy of this <br /> information.
