ACCESS AGREEMENT <br /> Ground Water Monitoring Well Installation <br /> 2123 Country Club Boulevard, Stockton, California <br /> As the property owner of 2123 Country Club Boulevard, I (Mr. Ron Berberian) do hereby grant <br /> permission to Advanced GeoEnvironmental, Inc. (AGE) to install one 2-inch diameter ground <br /> water monitoring well at 2123 Country Club Boulevard, Stockton, California. The investigation is <br /> being performed under the directive of the San Joaquin County Public Health Services <br /> Environmental Health Department (PHS-EHD). <br /> The scope of work for investigation at 2123 Country Club Boulevard will include drilling an 8-inch <br /> diameter borehole utilizing a truck mounted drill rig to an approximate depth of 25 feet below site <br /> grade(bsg)and installation of a two-inch diameter,perforated polyvinylchloride(PVC)ground water <br /> monitoring well within the borehole. <br /> Prior to performance of any field operations, a one week notification to Mr. Ron Berberian and the <br /> tenants at 2123 Country Club Boulevard will be given. Drilling activities will be completed in <br /> approximately 4 hours. The area utilized for well installation activities at 2123 Country Club <br /> Boulevard shall be returned to its original conditions prior to drilling activities. <br /> All costs for performance of the subsurface investigation will be the sole responsibility of Boulevard <br /> Automotive Service located at 2151 Country Club Boulevard (Mr. John Parker). Mr. Parker will be <br /> solely responsible for all costs and expenses for investigation associated with contamination from <br /> previous operations by the Boulevard Automotive site. <br /> Ground water samples will be collected from the newly installed well at 2123 Country Club <br /> Boulevard on a quarterly basis and continued until the PHS-EHD directs otherwise. Copies of all <br /> testing and reporting for any investigation performed at the newly installed well at 2123 Country <br /> Club Boulevard will be forwarded to Mr. Ron Berberian. Furthermore, Mr. Berberian will be <br /> provided with a copy ofthe"No Further Action"letter from the appropriate regulatory agencies upon <br /> completion of investigation, if requested. <br /> Mr. Ron Berberian - property owner: 2123 Country Club Boulevard, Stockton, California <br />
