(AGE) is hereby granted permission to install one 2-inch <br /> diameter ground water monitoring well at 2120 Elmwood Avenue, Stockton, California. The <br /> investigation is being performed under the directive of the San Joaquin County Public Health <br /> Services Environmental Health Department (PHS-EHD). <br /> The scope of work for investigation at 2120 Elmwood Avenue includes drilling an 8-inch diameter <br /> borehole utilizing atruck mounted drill rig to an approximate..depth-af 25 fait-below site gra&(bsg` <br /> and installation of a two-inch diameter, polyvinylchloride (PVC) ground water monitoring well <br /> within the borehole.Enclosed with this access agreement is anus-built ground water motutorin2 we11: <br /> construction diagram. <br /> The property owner will be notified at least one week prior to performance of fief <br /> to drilling activities, the property owner shall specify the exact location for t: <br /> 2120 Elmwood Avenue. Drilling activities will be completed in approximate',,, <br /> utilized for well installation activities at 2120 Elmwood Avenue shall be retunne. <br /> conditions prior to drilling activities. <br /> All costs for performance of the subsurface investigation will be the sole responsibility of Boulevard <br /> Automotive Service located at 2151 Country Club Boulevard(Mr. John Parker). bir.Parker will be <br /> solely responsible for all costs and expenses for investigation associated with contamination from <br /> previous operations by the Boulevard Automotive site. <br /> Ground water samples will be collected from the newly installed well at 2120 Elmwood Avenue on <br /> a quarterly basis and continued until the PHS-EHD directs otherwise. The property owner of 2120 <br /> Elmwood Avenue will be provided with copies of all testing and reporting for any investigation <br /> performed at the newly installed well at 2120 Elmwood Avenue.
