Dec 30 98 10: 22a JC— and BETTY RUTRN 2C^ 943 7362 p. 1 <br /> ACCESS AGREEMENT <br /> Monitoring Well Installation <br /> 2150 Country Club Boulevard, Stockton, California <br /> Advanced GeoEnvironmental,Inc. (AGE)is hereby granted permission to install one ground water <br /> monitoring well and perform quarterly ground water sampling at 2150 Country Club Boulevard in <br /> Stockton, California. The investigation is being performed under the directive of the San Joaquin <br /> County Public Health Services -Environmental Health Division. <br /> The scope of work includes the completion of one ground water monitoring well utilizing a CME-55 <br /> truck mounted drill rig to an approximate depth of 30 feet below surface grade(bsg). The boring will <br /> constitute an eight-inch diameter borehole. Drilling activities will be completed in one (1)day. The <br /> property owner will be notified at least one week prior to performance of any field operations. <br /> All costs for performance of the subsurface investigation will be the sole responsibility of the <br /> property owners at 2151 Country Club Boulevard (Boulevard Automotive Service). Boulevard <br /> Automotive Service will be solely responsible for all costs and expenses for investigation and/or <br /> remediation of contamination at 2150 Country Club Boulevard which is associated with <br /> contamination from previous underground storage tank operations by Boulevard Automotive <br /> Service. <br /> Upon completion of field work, the area investigated at 2150 Country Club Boulevard will be <br /> returned to its original conditions prior to field work activities. <br /> The property owners of 2150 Country Club Boulevard will be provided with copies of all testing and <br /> reporting for any investigation performed on their property.
